Job Description:
A company is looking for a Community Health Worker to support
population health initiatives in assigned neighborhoods.
Key Responsibilities
Provide guidance and support to members through home visits and
care coordination
Assist with member engagement, onboarding, and referrals to
clinical care programs
Promote programs and coordinate meetings for community health
workers
Required Qualifications
High School Diploma or equivalent
3 years of work experience, including 1 year in community
involvement or customer service
Understanding of Medicaid programs and community resources
Community Health Worker certification within 6 months of hire
Comfortable conducting home visits and proficient in Microsoft
Office
