A company is looking for a PRN/Per Diem Social Worker/Therapist-
Remote.
Key Responsibilities
Collaborate with an interdisciplinary team to provide therapeutic
services to older adult patients
Participate in community education on the importance of behavioral
health for older adults
Facilitate treatment team meetings and ensure compliant
documentation of patient-centered treatment plans
Required Qualifications, Training, and Education
Licensure in Social Work or Counseling as recognized by the state
(e.g., LISW, LCSW, LPC, LMFT) - required
Commitment to enhancing the quality of life for patients through
kindness and accountability - required
