We are seeking a dedicated Occupational Therapist to join our team. The ideal candidate will provide quality care to patients across various settings, including home health, outpatient clinics, acute care facilities, schools, and geriatric centers.

*Responsibilities*
* Conduct patient assessments and develop individualized treatment plans
* Provide occupational therapy interventions to help patients achieve their goals
* Collaborate with healthcare professionals to ensure comprehensive care
* Perform discharge planning and follow-up care as needed
* Utilize case management skills to coordinate patient care effectively

*Skills*
* Experience in assisting patients in various settings such as post acute care, acute care, and geriatric centers
* Proficient in discharge planning and case management
* Strong knowledge of physiology related to occupational therapy
* Ability to work effectively with diverse patient populations

Job Types: Full-time, Contract

Pay: $60.00 - $65.00 per hour

Expected hours: 40 per week
