Position Summary: Our Client is seeking an RN (Registered Nurse) Case Manager, a healthcare professional coordinating patient care, ensuring they receive appropriate services and treatment throughout their medical journey. They work closely with doctors, nurses, and other healthcare providers to develop and manage care plans, monitor progress, and assist patients with navigating the healthcare system. Their goal is to improve patient outcomes, optimize care efficiency, and reduce healthcare costs by managing resources and ensuring proper follow-up care. They may work in hospitals, insurance companies, or home health settings. Key Responsibilities: Care Coordination : Develop and manage individualized care plans. Patient Advocacy : Ensure patients receive appropriate care and resources. Monitoring Progress : Track patient outcomes and adjust plans as needed. Collaboration : Work with healthcare teams to optimize care. Resource Management : Manage healthcare resources to reduce costs. Education : Educate patients and families on care plans and self-care. Discharge Planning : Coordinate smooth transitions from hospital to home or other settings. Required Qualifications: Active, unrestricted certification/licensure as an RN in California State.
